EMERGENCY CARDS
Emergency Cards are distributed to students the first week of school through their homerooms. Please complete and sign the card and return to the Nursing Office as soon as possible. It is VERY important that we have current phone numbers on file. We will not take cell phone numbers from the students for their safety and ours. Please call or e-mail any changes throughout the school year. (School Nurse Ph # 261-3354) Be sure to list any medical conditions as this is the only way we have of knowing if there are any changes in your student's health. USE OF ELEVATOR
If your student needs to use the elevator a physician's note is required. This note should include the specific time frame the elevator is needed. Students are not to leave class early to use the elevator but may leave late to the following class. Only exception is the end of the day for a bus. There is a $20.00 charge for any lost elevator fobs.
STUDENT VISITS COMMUNICATION
After 10 visits to the nurse a form will be mailed home that lists the general symptoms the student is complaining about. This form is a tool to help the nurse communicate with the parent (and Guidance as time out of class may affect grades). If your child takes any "as needed" medications we will list how many doses we have given.
SCREENS - Seeing and hearing are important for learning!
Height, weight and vision is done on all students in 9th-12th grade. Hearing is done on all 11th grade. If a student fails their vision or hearing test a referral form is mailed home. This referral form should be taken to their physician for a follow-up. Return the completed referral form to the Nursing Office. Lion's Club referral forms can be requested through the Nursing Office if help is needed financially for an eye exam and glasses. They do not pay for contacts.
KEYSTONE PEDIATRIC CLINIC AT CASHS
Keystone Pediatric Clinic is now open at CASHS from 8:00 am to 12:00 noon every day that school is open. They will see any student that has permission from their parents. (See FAQ that is attached to the Permission Forms) A Permission Form and HIPPA Form need to be completed and returned to the CASHS Nursing Office if you want your child to be seen by a Keystone Provider. These forms are good for one year. You can find these forms on our Health Forms page, English and Spanish.
